What is this form for?

To provide information about the introduction of low risk chemicals in a flavour or fragrance blend to support the submission of a pre-introduction report.

You don't need to include information about chemicals in the blend that are already on our Inventory and you are categorising their introduction as 'listed'.

Who should use this form?

If you are the introducer of chemicals in a flavour or fragrance blend who submitted a pre-introduction report and/or the chemical identity holder for the chemicals to be introduced in a flavour or fragrance blend.

Do you know the CAS name, IUPAC name or eligible INCI plant extract name for the chemicals in the blend?
Are all of the chemicals in the blend that meet the criteria for 'low-risk flavour or fragrance blend introductions' on the IFRA Transparency List?
You don't need to submit this form if you have already given us all of the information about those chemicals and the name of the blend has not changed since you gave us the information.
